There are two possible drawbacks: 1) blood sugar might be getting too low in extreme hunger; 2) hormonal balance might be perturbed out of wack. Sleeping with extreme hungry might risk hypoglycemia (too low blood glucose). If blood sugar went below 40 or 50 mg/dL, coma might set in and patient never wake up. This is a known risk for type I diabetic patient. Hormonal imbalance might change receptor response to some signals of metabolism and could lead to body weight gain or lose later on.
